1. Services Provided

We provide psychiatric evaluations, medication management, and related mental health services to individuals ages 6 and older. Services may be provided in person at our San Antonio office or via telehealth to patients located in Texas and New Mexico where we are licensed.

3. Patient Eligibility

To receive services, you must:

• Be located in Texas or New Mexico at the time of your telehealth appointment (if applicable)

• Complete required intake forms and consent documentation

• Provide accurate and truthful health and personal information

4. Appointments, Cancellations, and No-Shows

• Appointments may be scheduled through our booking platforms, by phone, or email.

• Please provide at least 24 hours’ notice for cancellations or rescheduling to avoid a late cancellation fee.

• Multiple no-shows or late cancellations may result in termination of services.

5. Telehealth

Telehealth sessions are conducted via a secure, HIPAA-compliant platform. You are responsible for having the necessary technology and a private, quiet location for your session. We reserve the right to reschedule if the session environment is not appropriate for clinical care.

6. Fees and Payment

• We accept certain insurance plans, self-pay, and sliding scale payments.

• Payment is due at the time of service unless otherwise arranged.

• You are responsible for any fees not covered by insurance.

7. Confidentiality and Privacy

Your personal and health information is protected in accordance with HIPAA and our Privacy Policy. We will not share your information without your consent, except as required by law.

8. Limitations of Liability

To the maximum extent permitted by law, Shiloh Oak Behavioral Health PLLC is not liable for any indirect, incidental, or consequential damages resulting from the use of our services, website, or telehealth platform.

9. Website Use

By using our website, you agree not to:

• Post or transmit harmful or unlawful content

• Attempt to gain unauthorized access to our systems

• Use our content without written permission
